The biggest obstacle to curing HIV is the latent reservoir — infected cells that hide from the immune system and from antiretroviral drugs, ready to reignite the virus if treatment stops. A cancer drug just showed it can shrink that reservoir in monkeys.

Researchers at Emory University, led by Dr. Mirko Paiardini, gave venetoclax — a BCL-2 inhibitor approved for certain blood cancers — to 24 rhesus macaques infected with SIV (the monkey equivalent of HIV), starting at the same time as antiretroviral therapy. BCL-2 is a protein that keeps cells alive; latently infected CD4+ T cells may exploit it to persist. Blocking it pushes those cells toward death. The work was published in Nature Microbiology.

What they found

Venetoclax-treated animals showed a rapid reduction in intact SIV DNA in their CD4+ T cells versus controls — a drop that was sustained through day 294 and seen in lymph nodes too. Strikingly, the effect persisted for months after the drug was stopped.

Why it matters — and the caveat

Using an already-approved medicine to attack the reservoir is appealing, because it could speed the path to human testing. But this is a preclinical study in macaques, and shrinking the reservoir isn’t the same as curing HIV — the virus can rebound from even a small remaining reservoir. Still, it’s a promising, concrete strategy for chipping away at the hardest part of the HIV-cure problem.
