Could a common vitamin deficiency make surgery hurt more? A new study suggests a link between low vitamin D and greater pain after an operation — an intriguing but preliminary finding that comes with important limits.
What the study found
Researchers at Fayoum University Hospital in Egypt ran a prospective observational study of 184 breast cancer patients undergoing mastectomy. Those with vitamin D deficiency (blood levels below 30 nmol/L) were about three times more likely to experience moderate-to-severe pain in the first 24 hours after surgery than patients with adequate levels. The deficient group also needed substantially more pain medicine — roughly 112 mg more tramadol after surgery and a bit more fentanyl during it — and had higher rates of postoperative nausea. The findings were published in Regional Anesthesia & Pain Medicine.
A little context on vitamin D
Vitamin D is best known for bone health, but its receptors are found throughout the body, including in cells that regulate inflammation and immune responses — systems intimately involved in how we perceive pain. Deficiency is extremely common worldwide. The researchers propose that vitamin D may influence pain through these inflammatory and immune effects, though — a key limitation — they did not actually measure inflammatory markers in this study, so the mechanism remains a hypothesis.
Why the caveats matter
Several cautions temper the finding. First, it is observational, so it shows an association, not causation: people with low vitamin D may differ in other ways (nutrition, overall health, sunlight exposure) that also affect pain. Second, it was a single-center study in a specific population — women having breast-cancer surgery — so it may not generalize. Third, the differences were concentrated in the moderate pain range (no severe pain was reported in either group), and the study lacked data on factors like anxiety, depression and sleep that strongly influence pain. Crucially, it did not test whether giving vitamin D supplements actually reduces pain.
Why it matters
Even with those caveats, the idea is appealing because it points to something cheap and easily fixable. The researchers suggest that correcting low vitamin D before surgery might help modulate postoperative pain — a hypothesis worth testing in a proper randomized trial, which is the only way to know whether supplementation truly helps. For now, it is a reasonable prompt to ensure vitamin D status is not overlooked in surgical patients, but not evidence that a supplement is a painkiller.
