Dosing the powerful antibiotic vancomycin is notoriously tricky — too little fails to treat the infection, too much can harm the kidneys. A wearable patch from Nutromics that tracks vancomycin levels in real time has now won the FDA’s Breakthrough Device designation.

How it works

The patch uses microneedles coated with synthetic DNA-based sensors (aptamers) that latch onto vancomycin molecules and generate an electrical signal, streamed via Bluetooth to hospital displays. Instead of periodic blood draws sent to a lab, clinicians get a continuous readout of the drug’s concentration.

Why it matters

Vancomycin treats life-threatening infections like sepsis, but roughly 20% of ICU patients on it develop antibiotic-induced acute kidney injury. Real-time levels could enable guideline-adherent dosing and cut those complications. Nutromics (founded 2017) estimates about 2.9 million U.S. patients a year receive vancomycin. “Real-time vancomycin concentrations, while fitting within existing workflows,” is the goal, said CEO Peter Vranes.

The caveat

Breakthrough Device status speeds FDA engagement but is not an approval. The patch is still unapproved: Nutromics plans hospital clinical studies in 2026 and a possible 2027 launch, pending FDA clearance and positive results. Not medical advice.
